Primary Care Physician (Internal Medicine & Family Medicine)Location: Stockton, CA Job Type: Full-Time Schedule: Monday - Friday, Day Shift Setting: OutpatientRoles & ResponsibilitiesProvide comprehensive primary care services, including assessments, diagnosis, treatment, and chronic disease management for adult and geriatric patients.Conduct physical examinations, review medical histories, order diagnostic tests, and develop individualized treatment plans.Collaborate with the interdisciplinary team (IDT) to coordinate and monitor participant care and overall health outcomes.Prescribe and manage medications, preventive care, and health promotion programs for patients.Educate patients and families on health maintenance, chronic conditions, and treatment plans.Supervise and collaborate with Nurse Practitioners and Physician Assistants while coordinating specialist referrals as needed.Maintain accurate medical documentation, participate in quality improvement initiatives, and ensure compliance with organizational policies and healthcare regulations.Qualifications & ExperienceDoctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.Active California medical license required.Current DEA registration required.Board Certified in Internal Medicine or Family Medicine is required.Minimum of 1 year of experience working with frail or elderly populations.Geriatric care experience or added qualifications in geriatrics required.Benefits401(k)Health, Dental, Vision & Life insuranceEmployee assistance programEmployee discountFlexible spending accountHealth savings accountPaid sick timePaid time offReferral programRetirement plan
