Registered Nurse (RN)Mystic Healthcare & Rehabilitation Center is seeking an experienced and compassionate Registered Nurse (RN) to join our clinical team. In this role, you will lead resident care delivery, oversee LPNs and CNAs, collaborate with physicians and interdisciplinary staff, and ensure that our residents receive safe, high-quality, person-centered care in a long-term care and rehabilitation environment.Key Responsibilities:Conduct comprehensive nursing assessments and develop individualized care plansAdminister medications, IV therapies, and treatments per physician orders and facility protocolsMonitor residents for changes in condition and initiate appropriate clinical interventionsSupervise and provide clinical direction to LPNs, CNAs, and support staff on the unitCommunicate with attending physicians, specialists, and care team members regarding resident status and care needsCoordinate admissions, discharges, and transfers, including completion of required clinical documentationPerform and oversee skilled nursing procedures including wound care, tube feedings, catheter care, and moreRespond to emergencies and serve as clinical lead during urgent situationsEnsure compliance with state and federal long-term care regulations and facility policiesMaintain accurate, timely, and thorough documentation in the electronic health recordEducate residents and families on care plans, conditions, and discharge instructionsParticipate in quality improvement initiatives and care conferencesQualifications & Requirements:Active Connecticut RN license in good standing (required)Associate's or Bachelor's degree in Nursing from an accredited program (BSN preferred)Long-term care, skilled nursing, or rehabilitation experience preferredCPR/BLS certification (or willingness to obtain upon hire)Strong clinical assessment, critical thinking, and leadership skillsAbility to supervise and mentor LPNs and CNAs effectivelyExcellent written and verbal communication skillsAbility to stand, walk, and lift up to 50 lbs for extended periodsExperience with electronic health records (EHR) preferredWhat We Offer:Competitive base pay: $45.00–$56.00/hr based on experienceGenerous shift differential payHealth, dental, and vision insurance optionsPaid time off and holiday payContinuing education and career advancement opportunitiesSupportive, team-centered work environment
