Ambulatory Clinical Pharmacist - Clinic-Based Care

Saint Francis Hospital in Evanston, IL is seeking an Ambulatory Clinical Pharmacist for per diem weekday shifts. You will provide patient care and advanced clinical pharmacy services in the ambulatory setting, collaborating with prescribers and managing medication therapy. You will document patient interactions across EMR platforms and support clinics with a focus on chronic disease management.
