SPEECH-LANGUAGE PATHOLOGIST – THE VILLAS AT SUNNY ACRES - THORNTON, CO Full‑time position at a skilled nursing facility providing long‑term care, post‑acute rehabilitation, and geriatric outpatient therapy. The role involves clinical development, career growth, and leadership training opportunities. Responsibilities Screen and evaluate patients with communication, cognitive, or swallowing disorders; develop plans of care per regulatory and clinical practice standards. Provide rehabilitative, skilled, and medically necessary treatment interventions for patients with communication, cognitive, and/or swallowing disorders. Evaluate treatment outcomes for effectiveness and modify services accordingly; make referrals based on outcomes. Offer consultation and counseling to patients, families, caregivers, and other service providers related to speech disorders. Prepare comprehensive discharge summaries of services provided, in compliance with regulatory requirements. Document all regulatory and clinically required information in the electronic documentation system; maintain patient records per company policy. Qualifications Licensed Speech‑Language Pathologist.
