This is a 13-week contract position.

Schedule

Shift: 8:00 AM - 5:00 PM, with every other weekend rotation or a permanent schedule option based on agency needs.

Job Description

We are seeking a dedicated Registered Nurse for home care services. The ideal candidate will manage a case load of 20-25 patients, conducting approximately 6 visits per day. Candidates must be flexible and prepared to respond to additional visit requests as needed.

Responsibilities

• Manage home care patients, focusing on their individual needs and care plans.
• Document visits utilizing Electronic Medical Records (EMR), specifically EPIC and OASIS.
• Perform nursing tasks including IV therapy, administering tube feeds, managing tracheostomies, and caring for wound and ostomy patients.
• Collaborate with healthcare professionals to ensure optimum patient outcomes.
• Communicate effectively with patients and their families regarding care and treatment options.

Required Experience / Certifications / Licensure

• At least 2 years of nursing experience in home health or a similar setting.
• BLS Certification is required.
• Experience with EPIC / OASIS is preferred.
• Proficient in managing complex medical care including IVs, trachs, and wound care.
