Position Summary
This position will be a floater between the behavioral health programs. The Case Manager is responsible for coordinating, monitoring, and supporting clients throughout their treatment experience at Nat-Su Healthcare. This role ensures continuity of care, compliance with program and court requirements, and progress toward individualized goals. The Case Manager serves as a liaison between clients, clinical staff, program leadership, and external stakeholders while maintaining professional, ethical, and regulatory standards.
Job Duties and Responsibilities
Client Care & Coordination
- Serve as the primary case management contact for assigned clients.
- Monitor client compliance with treatment expectations, court requirements, and program guidelines.
- Assist clients with developing goals and meeting session expectations.
- Support clients in identifying and addressing barriers to treatment, stability, and successful program completion.
Meeting & Program Participation
- Attend Program Staff meetings and weekly Clinical meetings.
- Attend Monthly AP&P meetings.
- Attend Monthly Case Management meetings.
- Attend All-Staff meetings once per month, in person when possible.
Intake & Incoming Clients – to be done within 24 hours of intake
- Welcome clients to Nat-Healthcare
- Complete all intake paperwork not completed by Admissions
- Complete all Case Management Assessments and intake check list.
- Schedule weekly case management meetings.
Ongoing Case Management Services
- Maintain accurate documentation in the EHR.
- Complete required court and referral documentation.
- Report non-compliance and incidents.
- Complete documentation within 24 hours.
Appointment & Resource Support
- Coordinate medical, dental, court, and community appointments.
- Support Transportation planning.
- Provide community resource referrals.
Other Duties
- Additional duties may be assigned.
Education and Experience
- High School Diploma or equivalent preferred.
- 6 months of Behavioral Health or Human Services experience.
- Case Manager Certification preferred.
Workload Expectations
- Work Schedule Full-Time Monday – Friday
- Residential Treatment (RTC): 12-15 clients depending on other responsibilities.
- Outpatient Services: Up to 20 clients.

Physical Requirements To comply with the Rehabilitation Act of 1973 the essential physical, mental, and environmental requirements. Candidates must be able to successfully perform all functions of their job with minimal supervision.
This position is subject to Native American/Alaskan Native preference.
