Huntsville Memorial Hospital is seeking an RN Case Manager to support physicians and the care team in coordinating patient care, maximizing clinical outcomes, and controlling costs. Under general supervision, the RN Case Manager will manage a designated caseload, facilitate pre-certifications and discharge planning, communicate with payors, and drive performance improvements across the continuum of care. #J-18808-Ljbffr
