Home Health RN Case Manager Meaningful Patient Care

AccentCare, Inc. is seeking a Home Health Registered Nurse Case Manager in Desoto, TX. The role involves managing patient care plans from admission through discharge, coordinating with RNs, LPNs, and CHHAs to deliver the plan of care and ensuring quality patient care.

The RN will perform initial assessments, monitor medications and document care, with a schedule of Monday to Friday 8-to-5 and monthly on-call. Compensation reflects annualized pay with a per-point basis component.
