Diabetes Nurse Educator

Hartford Healthcare seeks a compassionate Diabetes Nurse Educator to provide evidence-based diabetes self-management education for patients and families. In this role, you will assess learning needs, develop individualized education plans, and lead group classes on glucose monitoring, nutrition, and medication management. You’ll collaborate with an interdisciplinary team to improve outcomes, support technology use (CGM, pumps), and contribute to quality initiatives.
