We are seeking a skilled and empathetic Social Worker (SW) to join our team. In this vital role, you'll help patients and families cope with the emotional, social, and practical challenges that come with a terminal diagnosis—bringing comfort and clarity during their most vulnerable moments.What You'll DoConduct psychosocial assessments of patients and caregiversProvide counseling to patients and families regarding emotional stress, coping, grief, and advance care planningAssist with advance directives, funeral arrangements, and navigating legal/financial decisionsIdentify and coordinate with community resources, agencies, and support organizationsEducate staff and families about cultural and psychosocial needsDocument patient conditions, interventions, and psychosocial support providedServe as a liaison between families and care teams to ensure coordinated, holistic careParticipate in interdisciplinary team meetings to develop individualized care plansProvide crisis intervention and support for safety concerns (neglect, abuse, self-harm)Participate in quality improvement initiatives and continuing educationParticipate in on-call support as neededQualificationsBachelor's degree in Social Work (BSW) requiredMaster's degree (MSW) or LCSW strongly preferredAt least 1 year of experience in social work, preferably in hospice, home health, or a community settingKnowledge of end-of-life care, grief support, and cultural sensitivityStrong written and verbal communication skillsSelf-motivated, organized, and able to work independentlyCurrent Basic Life Support (CPR) certificationValid driver's license, auto insurance, and reliable vehicle requiredPlease note: Legacy Hospice is a drug-free workplace.
