Quality Improvement Coordinator

Responsibilities: Conduct diagnostic evaluations to assess patient needs and develop individualized care plans Collaborate with healthcare providers Monitor patient progress and adjust care plans as needed Maintain accurate medical records and documentation Work with diverse patient populations including geriatrics and individuals with complex medical conditions Complete chart audits for compliance Educate staff for the purpose of improving compliance Qualifications: Familiarity with medical terminology and procedures Strong communication and interpersonal skills Ability to work effectively in a multidisciplinary team environment Experience working with electronic health records systems Requirements: Previous experience as an ADvantage case manager not required Licensed as an LPN or RN OR Bachelor's degree required One year working with elderly adults required #J-18808-Ljbffr 6AM City, LLC
