Summary Position type: Non‑exempt, full‑time or part‑time, hourly role. Hourly rate: $9.00. Responsibilities Provide direct support to 4–6 individuals in residential settings or up to 10 in periodic programs. Deliver support per individual service or program plans, including socialization, health maintenance, medication administration, skill/behavioral development. Assist with daily activities such as personal care, housekeeping, recreational activities, transportation, community integration, shopping, and citizenship activities. Accurately document progress and activity logs, maintain confidentiality. Make cost‑effective purchases within the individual’s budget and preferences. Maintain professional relationships with individuals, families, guardians and case managers; follow Sevita Customer Service Standards. Report abuse, neglect, medical or behavioral incidents in accordance with policies and regulations. Administer or support self‑administration of medications and document delivery; follow controlled medication procedures. Medically monitor individuals, document concerns, communicate with nurse or supervisor, and call 911 in emergencies. Use medical equipment correctly and report malfunctions. Prepare or assist in preparing meals, respecting special diets and individual choice. Participate in orientation, ongoing training, monthly staff meetings. Adhere to employee scheduling, arrive on time, stay until replacement arrives, and cover shifts when required. Work cooperatively as part of a team, communicate openly, and maintain workplace safety. Transport individuals, ensuring safe driving, use of seat belts, lifts, and wheelchair ties; report accidents immediately. Perform housekeeping and environmental safety checks; report hazards. Perform minor repairs and yard maintenance as directed. Assist with bathing safety checks and participate in safety drills. Perform any other duties as assigned. Maintain Mobile Punch Authorization for timekeeping. Qualifications High school diploma or equivalent (preferred). At least six months of experience in human services (preferred). Minimum age 18 years. Working knowledge of computers. Valid driver's license in good standing; vehicle registration and insurance if providing transportation. Compliance with all required state trainings and certifications. Must be able to lift up to 100 lbs occasionally, 50 lbs frequently, and 20 lbs constantly.
