Position Summary

St. Augustine Health & Rehabilitation Center is seeking compassionate and dependable Registered Nurses to join our nursing team on a PRN basis.

The Registered Nurse will provide high-quality nursing care in a long-term care and rehabilitation setting while supporting each resident’s physical, emotional, and social well-being. This position also works closely with physicians, therapists, nursing staff, and other healthcare professionals to promote positive resident outcomes.

Essential Responsibilities

Provide direct nursing care in accordance with physician orders, resident care plans, and facility policies.
Assess residents and monitor changes in condition.
Administer medications and treatments accurately and safely.
Develop, implement, and evaluate individualized care plans.
Notify physicians, families, and appropriate team members of changes in resident condition.
Collaborate with physicians, therapists, and other healthcare professionals.
Maintain accurate and timely clinical documentation.
Supervise and support Certified Nursing Assistants and other direct care staff.
Follow infection-control procedures and applicable state and federal regulations.
Promote a safe, respectful, and resident-focused environment.

Qualifications

Current and unrestricted Registered Nurse license in the State of Florida.
Current BLS/CPR certification.
New graduates are encouraged to apply.
Long-term care, skilled nursing, rehabilitation, or geriatric nursing experience is preferred.
Strong assessment, clinical, communication, and organizational skills.
Ability to work effectively as part of an interdisciplinary team.
Compassionate, dependable, and committed to excellent resident care.
