We're on the hunt for a dynamic, organized, and compassionate Service Coordinator to be the heartbeat of our care team. You'll be the master of schedules, the champion of quality care, and the trusted support for both clients and caregivers. If you love solving problems, building relationships, and keeping things running smoothly, this is the role for you!

Hours: Full Time: In office Monday-Friday 8am - 5pm. On call every other week from 5pm - 8am and 24hrs Saturday - Sunday.

What You'll Do:

• Create and manage caregiver schedules with precision and care
• Ensure uninterrupted service by coordinating coverage for client needs
• Conduct monthly wellness calls and in-home visits
• Be the go-to for client updates, service changes, and support
• Supervise and motivate direct service employees
• Maintain a professional, welcoming office environment
• Uphold confidentiality and compliance with HIPAA and Medicaid/Medicare regulations

What You Bring:

• High school diploma or equivalent
• At least 2 years of industry experience
• Strong interpersonal, organizational, and communication skills
• Tech-savvy with Microsoft Word, Excel, and scheduling software
• Reliable transportation
