RN - Hospice

Registered Nurse (RN)The Registered Nurse will provide holistic, family-centered care across treatment settings to enhance the quality of life for patients and their families. This role involves assessing various needs and developing individualized plans of care to meet those needs.Key ResponsibilitiesIdentify patients' and families' physical, psychosocial, environmental, safety, and developmental needs.Document nursing assessments, care interventions, goals, and outcomes achieved from interventions.Assess all aspects of the patient's pain and develop tailored pain management plans.Anticipate, prevent, and treat undesirable or secondary symptoms.Participate in interdisciplinary team meetings to establish, review, and revise plans of care.Maintain accurate and up-to-date patient records, ensuring compliance with regulations, certification standards, and ethical norms.Educate patients, caregivers, and families about interventions, medications, and potential side effects.Implement care strategies that support comfortable and dignified end-of-life processes.Support patients' and families' individual spiritual and cultural beliefs.Consult and collaborate with various team members involved in patient care.Provide care in accordance with National Hospice and Palliative Care Organization standards.ScheduleThis position requires flexibility, with responsibilities including night and weekend shifts as needed.
