Case Manager

Summary of Position: The purpose of this position is to provide leadership to the clinical team in planning a patient’s discharge. The case manager is the member of the clinical team who is responsible for identifying the needs of the patient as they prepare for discharge, and is responsible for identifying resources in the community that may be used to meet those needs. Case managers assist the patient in breaking the cycle of homelessness by connecting them to any available housing options for which they may qualify. This is a safety sensitive position.Essential Duties:Works as part of the clinical team with the primary responsibility of discharge planningAttends daily team huddles when in clinic, clinic staff meetings, and team meetingsMaintains appropriate documentation in the medical record so that the case management plan for each patient is easily accessibleWorks with each patient on a comprehensive plan for discharge to permanent housing optionsFosters empowerment and recovery by connecting the patient with community agencies to obtain services such as vocational rehabilitation, mental health and substance abuse services, job readiness and parenting skillsAccompanies patients on site visits with the goal of clarifying the discharge planCounsels patients on entitlements for which they may qualify and facilitates applicationsKeeps the clinical team updated on progress of each patient towards dischargeCollects data accurately for all grants and projects, as directed by supervisorSupervisory Responsibility:NoneQualifications:Associates Degree or equivalent experience required; BSW preferred3 – 5 years of experience preferredStrong interpersonal skills with an ability to work effectively with a wide range of people, teams, managers, supervisors, and vendorsExperience in working with culturally diverse, chronically homeless, and medically needy individuals preferred.Demonstrates excellent oral and written communication skillsBilingual skills – Spanish highly desirableMust demonstrate critical thinking, problem-solving, and organizational and time management skillsPossess and maintain a valid Arizona driver’s license and has use of privately owned vehiclePhysical and Mental RequirementsPosition requires extended periods of sitting and standing including bending and reachingHandles stress in a positive mannerAbility to be flexible and to multitaskDemonstrated interest in working with an underserved population #J-18808-Ljbffr Circle-the-Cit
