Requirements
- Current state license as an Occupational Therapist - Required
- Proof of current CPR certification - Required
- Current driver's license - Required
- Reliable transportation and insurance - Required
- Two years of experience as an Occupational Therapist in an acute care or rehabilitation setting - Required

Roles & Responsibilities
- Provide coordinated care to patients of all ages in a home care environment.
- Plan, implement, and evaluate individualized patient care plans to assist patients in developing, regaining, or maintaining their well-being and ability to perform daily activities.
- Evaluate patient occupational performance and assess their physical, psycho-social, and cognitive needs.
- Collaborate with an interdisciplinary team and communicate pertinent patient information while respecting confidentiality.
- Document patient care including assessments, progress notes, and plans of care; maintain accurate records.
- Adapt therapeutic interventions by assessing the need for and use of adaptive equipment and modifications to the patient's environment.
