Your Responsibilities

• Lead and coordinate care throughout the entire patient journey.
• Integrate patient care among clinicians, caregivers, and providers.
• Manage physician orders, documentation, and workflows within the electronic medical record (EMR) system.
• Collaborate with Intake to facilitate timely admissions and minimize referral delays.
• Conduct ongoing patient assessments and ensure alignment with the care plan.
• Review clinical reports (such as occurrences and infection control) and take necessary actions.
• Facilitate case conferences, recertifications, and discharge planning.
• Ensure adherence to clinical, state, and federal regulations.
• Act as a key liaison among physicians, patients, families, and internal teams.
• Support care delivery through in-home visits as required.

Qualifications We Seek

• Active RN license in the state of practice.
• Graduate of an accredited nursing program (BSN preferred).
• Experience in home health is highly preferred.
• Background in care coordination, case management, or clinical leadership is advantageous.
• Strong clinical judgment and ability to manage multiple priorities effectively.
• Confident communicator capable of collaborating across various disciplines.
• Comfortable working independently in a fast-paced, field-based setting.
