- Lead and coordinate care throughout the entire patient journey.
- Integrate patient care among clinicians, caregivers, and providers.
- Manage physician orders, documentation, and workflows within the electronic medical record (EMR) system.
- Collaborate with Intake to facilitate timely admissions and minimize referral delays.
- Conduct ongoing patient assessments and ensure alignment with the care plan.
- Review clinical reports (such as occurrences and infection control) and take necessary actions.
- Facilitate case conferences, recertifications, and discharge planning.
- Ensure adherence to clinical, state, and federal regulations.
- Act as a key liaison among physicians, patients, families, and internal teams.
- Support care delivery through in-home visits as required.
- Active RN license in the state of practice.
- Graduate of an accredited nursing program (BSN preferred).
- Experience in home health is highly preferred.
- Background in care coordination, case management, or clinical leadership is advantageous.
- Strong clinical judgment and ability to manage multiple priorities effectively.
- Confident communicator capable of collaborating across various disciplines.
- Comfortable working independently in a fast-paced, field-based setting.
- Demonstrated ability to manage complex patient cases and coordinate across teams.
- Confidence in making clinical decisions and driving positive outcomes.
- Strong organizational skills with meticulous attention to detail in documentation and compliance.
- Proactive in identifying issues and taking appropriate actions.
