Physical Therapist - Travel Contract

Minimum Requirements Doctoral, Master's, or Bachelor's Degree in Physical Therapy from an accredited program approved by the APTA. Active Ohio Physical Therapist license required to start the assignment. BLS certification from AHA or ARC may be required. Minimum 1 year of clinical experience in home health or hospice recommended; experience with home health patient evaluations, plan of care development, and skilled documentation preferred. Comfort with electronic medical records and productivity expectations. Client requires weekly productivity target of 30 points and a weekly hour guarantee of 36 hours. Valid driver's license and reliable transportation for patient visits. Mileage between visits will be reimbursed. Willingness to follow client dress code: gray or black scrubs. Preferred: prior experience working with LHC-style documentation or familiarity with LHC Group workflows. Preferred: hospice experience and skills in mobility, balance, and fall-prevention interventions. Assignment Details Facility Type: Home Health Shift Name: Days Shift Details: Days 0800-1600 (8:00 AM to 4:00 PM) Weekly Hours: 36 hour guarantee per client guidance. Productivity Expectation: 30 points per client guidance. On Call/Weekend Requirement: No. Mileage Reimbursement: Yes, between patient visits from first to last patient. The area is near Mohican State Park (approximately 25 miles east), which provides hiking, mountain biking, and canoeing on the Mohican River for adventurous outdoor recreation. Local amenities include shopping at The Shoppes at Richland County and a variety of dining options along Lexington-Springmill Road, supporting a suburban lifestyle with commuter access to nearby cities. Public transit options are regional; most clinicians use personal vehicles for daily travel between patient visits and surrounding communities.
