Responsibilities:

• Provide direct patient care, including administering medications, wound care, and vital sign monitoring
• Develop and implement individualized care plans in collaboration with the healthcare team
• Monitor and report changes in patient condition to the nursing staff and physicians
• Participate in discharge planning and education for patients and families
• Maintain accurate and thorough patient records
• Collaborate with interdisciplinary team members to ensure seamless care transitions
• Perform other duties as assigned by the nursing staff and management

Requirements:

• Currently licensed as a Licensed Practical/Nurse (LPN) in the state of Wisconsin
• High school diploma or equivalent required
• 1-2 years of experience as an LPN preferred
