Licensed Practical Nurse

Berkshire Post Acute in Sacramento seeks an LPN to provide compassionate bedside care for post‑acute, rehab, and long‑term residents. The LPN will administer medications, perform treatments, monitor vital signs, document in the EHR, and support individualized care plans in collaboration with RNs, CNAs, and therapists. You’ll communicate closely with residents and families, follow infection‑control and safety protocols, and help maintain a respectful, dignified environment that reflects our patient‑first, team‑oriented culture.
