This position focuses on skilled nursing visits to patient homes, ensuring continuity of care and positive clinical outcomes.Schedule:Full-time, 13-week contractMonday Friday, 8:00 AM 5:00 PMCaseload: 2532 points per week (typically 35 patient visits per day)Daily travel required within the assigned territory (mileage reimbursement provided)Qualifications:Active Registered Nurse license in good standingAt least 1 year of recent field-based Home Health experienceProficient with Home Care Home Base (HCHB) electronic documentation systemProven experience conducting OASIS assessmentsExcellent clinical assessment, time management, and organizational abilitiesReliable transportation, valid drivers license, and current auto insuranceAbility to work independently and maintain effective communication with the care teamBSN preferredFamiliarity with managing 2532 weekly points and Medicare/home health compliance is a plusKey Responsibilities:Conduct comprehensive OASIS assessments, develop, and implement individualized care plansDeliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease managementMonitor patient progress and communicate changes to physicians and interdisciplinary team membersEducate patients and families on treatment plans, disease management, and medicationsDocument care promptly and accurately within the HCHB systemCollaborate with therapists, social workers, and aides to coordinate servicesParticipate in staff meetings, case conferences, and ongoing educationAdhere to organizational and regulatory standards for documentation and quality of careBenefits include weekly pay, medical/dental/vision insurance options, 401(k) plans, 24/7 support, mileage reimbursement, and the potential to extend or renew your contract.Sunbelt is an equal opportunity employer.
