Duties and Responsibilities Serve on an ACCS Integrated Team by attending regularly scheduled meetings and presenting psychosocial history, mental status, diagnosis, treatment provided, and treatment plans for assigned cases. Assist in developing, understanding, and implementing treatment plans for assigned caseloads, involving the person served and significant others as appropriate. Assess ongoing needs of assigned persons served through supportive counseling, record review, home visits, and regular contact to ensure appropriate treatment recommendations. Act as an advocate and liaison between persons served and primary/other health‑care providers regarding medical and medication issues. Provide rehabilitative interventions to help persons served build skills and access resources, incorporating SAMHSA’s 8 Dimensions of Wellness. Read and communicate all pertinent information during daily rounds and staff meetings, informing supervisors of critical information. Transport or arrange transportation for persons served who need that level of service. Assess mental status and assist persons served with coping strategies, natural supports, and safety planning when necessary. Involve assigned persons served and significant others in treatment planning, ensuring plans are agreed upon with signatures or LAR. Maintain timely records in EMR and paper record, completing all required forms to ensure compliance with DMH regulations and Rehab Option requirements. Provide primary nursing care by assessing health status, recording data, evaluating outcomes, and adjusting care with other service providers. Transcribe and ensure implementation of physicians’ orders. Conduct self‑medication readiness assessments at admission and during care transitions. Complete physical health assessments and verify medications at admission, updating as needed. Required Qualifications Knowledge and ability to plan, organize, and participate in general and specialized nursing care of psychiatry patients. Knowledge of theories and principles of nursing, including psychiatric and mental health terminology. Ability to direct paraprofessional workers and maintain effective working relationships with other workers. Knowledge of principles and techniques of psychopharmacology, including medication administration, observation, reporting, and recording. Ability to read and interpret physicians’ orders and treatment plans. Ability to make decisions and act quickly in emergencies. Ability to understand, interpret, and apply psychosocial data and nursing knowledge to make appropriate recommendations. Preferred Qualifications Prior experience working with people with chronic mental illness. Experience working in a Community Mental Health Setting. Special Requirements Current and valid Massachusetts Class D Motor Vehicle Operator’s license or equivalent from the state of residence for travel.
