Experience in discharge planning within an acute care hospital for a minimum of 6 months is required. Candidates should ideally come from a large organization or teaching hospital setting, specifically with 500 beds or more. Experience in Obstetrics and Pediatrics is appreciated but not required. The position requires a commitment to working 40 hours per week, which may consist of 4 ten-hour shifts or 5 eight-hour shifts, depending on operational needs. Day shift hours are Monday through Friday.
