Senior Case Manager

The University of Oklahoma is an equal opportunity institution ( Position Overview The College of Nursing is seeking a dedicated and experienced Senior Case Manager to join our Case Management Department supporting home and community-based waiver programs. This position plays a critical role in coordinating services for aging and disabled populations while helping individuals maintain independence and quality of life within their homes and communities. Key Responsibilities Manage a full-time caseload and serve as an experienced resource within the department, providing guidance, support, and leadership to team members while ensuring high-quality case management services are delivered in accordance with organizational standards and program requirements. Work independently under limited supervision and be responsible for all aspects of the case management process, including admissions, assessments and reassessments, service planning, monitoring, coordination of care, and discharge planning. Develop and maintain meaningful relationships with clients, caregivers, providers, and interdisciplinary teams. Qualifications Strong critical thinking, communication, and organizational skills.
