DREAM Enrichment Program Case Manager

The DREAM Enrichment Case Manager is responsible for facilitating and implementing an individualized and outcome-based planning process for each person receiving services and for assisting the Director of Waivered Services to ensure that outcomes are being implemented correctly and that data being collected is accurate.
