Full-Time Physical Therapist (PT)

We are seeking a full-time physical therapist (PT) for a very flexible home health position. This role offers meaningful one-on-one patient care, manageable territories, and excellent compensation—without on-call requirements.

Key Responsibilities:

• Provide physician-ordered physical therapy services in patients' homes
• Evaluate patient function and develop individualized plans of care
• Complete initial evaluations, reassessments, and plan-of-care updates every 30 days
• Supervise physical therapy assistants and home health aides as appropriate
• Educate patients and caregivers on exercises and home care routines
• Collaborate with physicians and the interdisciplinary care team
• Complete timely and accurate clinical documentation, including OASIS
• Participate in quality initiatives, in-services, and care conferences

Requirements:

• Bachelor's, master's, or doctorate degree in physical therapy
• Active physical therapy license in the state
• Valid driver's license and reliable transportation
• Must be a U.S. citizen or green card holder (no exceptions)
