You'll support patients with daily care needs, monitor their condition, and help ensure safe, high-quality home health services.Responsibilities: Provide nursing care according to the RN-developed Plan of Care. Monitor and document changes in patient condition; report to RN/physician as needed. Assist with activities of daily living to promote patient independence. Educate patients and caregivers within LPN/LVN scope of practice. Maintain accurate EMR documentation. Participate in case conferences and coordinate care with the home health team. Follow infection-control, HIPAA, and company policies.Requirements: Current LPN/LVN license in the state of practice. Graduate of an accredited practical/vocational nursing program. Minimum 1 year acute care experience preferred. Current CPR/BLS certification. Valid driver's license, reliable insured vehicle. Strong communication, clinical judgment, and ability to work independently. Physical Requirements Ability to lift/transfer patients and perform physical caregiving tasks.
