Home Care Registered Nurse

University Home Care Inc is seeking a compassionate and skilled Home Care Registered Nurse to join our team in Macomb, MI. In this role, you will provide high-quality nursing care to patients in their homes, ensuring their comfort, safety, and overall well-being. You will work closely with patients, families, and healthcare professionals to develop and implement personalized care plans that promote health and independence.
Responsibilities

• Conduct thorough patient assessments and develop individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor patient progress and communicate changes in condition to healthcare providers.
• Provide education and support to patients and their families regarding health management.
• Maintain accurate and timely documentation of nursing care and patient records.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Follow all safety protocols and maintain compliance with healthcare regulations.

Requirements

• Valid Registered Nurse (RN) license in the state of Michigan.
• Previous experience in home health care or a related nursing field preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and manage a caseload effectively.
• Reliable transportation to travel to patient homes within Macomb, MI.
