Responsibilities Conduct case management, linkages, and referrals to needed supports in the community. Act as a liaison between the program and community partners to facilitate referrals and coordination of care for WTW clients. Assist clients and families with SSI applications; obtaining childcare; connecting with primary care, arranging medical and dental appointments; filling out and completing applications and forms for services. Assist clients in arranging transportation and teach how to use public transportation, including learning routes. Provide individual and group rehabilitation and family activities. Assist clients in meeting service‑plan goals, prioritizing areas of need and steps needed to reach goals as well as identified strengths. Assist clients with educational and vocational goals, conduct employment assessments and help link clients to schools, volunteer or employment opportunities. Work with businesses and agencies in the community to build relationships that support client job development and self‑sufficiency. Provide client transportation as needed. Additional Qualifications and Skills Minimum two years of experience working in the mental health field Bilingual Spanish preferred, but not required Intermediate computer skills in Microsoft Office, including Outlook, Word, and Excel Valid California driver’s license in good standing, personal vehicle, proof of insurance, and eligibility to serve as an approved CRF driver (required for essential functions) Able to pass criminal/background clearance CRF inspires and mentors the next generation of behavioral health professionals!
