·Ensure for the personal care and appearance of all clients.

·Ensure that the living environment remains safe, comfortable, clean and supportive to the clients’ needs.

·Implement daily program activities as directed on the facility program schedule.

·Have knowledge of and implement the Individual Service Plan for each client. Must be familiar with all objectives as written by the facility Interdisciplinary Team.

·Communicate with the facility QIDP regarding client progress and needs. 

· Must have knowledge of clients’ rights and must protect and advocate for these rights. 

·Must attend all required inservice training sessions and staff meetings. A minimum of 3 hours of inservice training per month is required.

·Communicate all health related issues in a timely fashion to the licensed nurses and/or Director of Nurses.

·Must have knowledge of how to respond to emergency situations.

·Monitor and provide proper maintenance for clients’ personal properties, including clothing, toiletries and other personal belongings.

·Must complete all documentation as required. Documentation includes, but is not limited to charting for Activities of Daily Living, IPP objectives and behavior data.

·Must report to work promptly and as scheduled.

·Must appear at work well groomed and appropriately dressed.

·Must observe all safety, health and infection control standards.

·Must adhere to facility policy and procedures at all times.

·Perform other duties as assigned.
