What You'll Do

As a Physical Therapist or Occupational Therapist, you'll play a vital role in helping residents regain strength, confidence, and independence.

Responsibilities include:

• Perform comprehensive evaluations and develop individualized treatment plans

• Deliver evidence-based therapy services in a skilled nursing and rehabilitation setting

• Monitor resident progress and adjust treatment plans to maximize outcomes

• Collaborate closely with nursing, physicians, and the interdisciplinary care team

• Educate residents and families on treatment goals and therapeutic techniques

• Maintain accurate and timely clinical documentation

• Ensure compliance with all state, federal, and professional practice standards

• Help residents improve mobility, function, independence, and overall quality of life

What We're Looking For

• Current Colorado Physical Therapist (PT) or Occupational Therapist (OT) license, or eligibility for Colorado licensure

• Graduate of an accredited Physical Therapy or Occupational Therapy program

• Skilled Nursing Facility (SNF) or Long-Term Care (LTC) experience is preferred but not required

• Strong clinical reasoning, communication, and documentation skills

• A compassionate clinician who enjoys working collaboratively and making a lasting impact
