Job Summary Pharmacist. Full-time, Contract – 4‑10’s. In‑Office | Peridot, AZ, United States. The Opportunity Period of Performance One year – twelve (12) months from the date of the award plus four option years. Place of Performance Lawton Indian Hospital. Lawton, OK 73507. Work Schedule A. The Contractor shall provide on‑site pharmacist services and coordinate patient care with all appropriate departments. B. Normal shifts for the Pharmacy Department consist of 8‑hour shifts. Working hours are shifts between the hours of 8:00 am to 5:00 pm with a 1‑hour lunch break or 8:00 am to 4:30 pm with a half‑hour lunch break, depending on the site assigned. Monday through Friday shifts (7:00 am to 4:00 pm, 9:00 am to 6:00 pm or 1:00 pm to 10:00 pm) and 10:00 am to 6:00 pm Saturday and Sunday (at select facilities). No federal holidays are worked. Professional Qualifications Graduate of an accredited College of Pharmacy. Documentation of annual Continuing Pharmacy Education required for licensure. Current unrestricted license to practice in a state, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States at all times. Resume of past professional experience. Documentation of a Board Certification in Ambulatory Care Pharmacy (BCACP) or Pharmacotherapy (BCPS) from the Board of Pharmacy specialties, or completion of a Post Graduate Year One residency. Demonstrated experience in management of patients with diabetes within the context of a collaborative practice agreement. #J-18808-Ljbffr
