Registered Nurse/Case Manager

Schedule & Location Work Schedule: 40 hours per week, 5 x 8-hour day shifts (0800-1630) Location: Georgetown, Washington, DC Field Visits: Required 2 days per week (mileage reimbursed) On Call: None Weekend Requirement: None Requirements Pending DC RN license accepted but must have license prior to start date Basic Life Support (BLS) certification 2 years of experience in Acute Hospital, Long Term Acute Care/Rehab/Skilled Nursing Experience in Care coordination, Discharge Planning, Admission Criteria Knowledge of InterQual Criteria and Milliman Guidelines Regulatory knowledge: CPT codes, Department of Health DRG, HEDIS measures, NCQA, OSHA procedures, and JCC First-time travelers accepted with a strong background Unit Information Chronic diagnoses: High Risk Pregnancy, Postpartum, DM, CHF, COPD, Asthma, obesity/overweight, Behavioral Health Responsibilities include making calls to enrollees, completing comprehensive assessments, developing care plans, and ongoing management within the EMR Dress Code: Business casual #J-18808-Ljbffr TALENT Software Services
