RN Case Manager – Home Health Hospice RN
Agency: Conflux Systems Inc
Pay: $60/hr on w2
Shift schedule is M-F, 8-5. On-call and weekend rotation.
Location: Missoula, MT
Contract Duration: 13 weeks
Start Date: 5/27/2026

Seeking an experienced RN Case Manager to provide skilled nursing care and coordinate patient services in a home health setting. Responsibilities include patient assessments, care planning, wound care, medication management, patient education, documentation, and coordination with interdisciplinary teams.

✅ Active Montana RN License required
✅ Home Health / Acute Care / Hospice experience preferred
✅ HCHB experience strongly preferred
✅ Valid Driver’s License & Reliable Transportation required
