CHMHA has a fantastic opening for a Registered Nurse/Licensed Practical Nurse (RN/LPN). Our RN/LPN will work as part of a team to provide mental health services in 24-hour residential care facilities. They will coordinate with staff prescribers and outside primary care providers as needed. At Community House, we value a strong work-life balance and aim to provide the best support for our workers. SCHEDULE The schedule for this shift is Monday-Friday, 35 hours per week. RESPONSIBILITIES Perform basic physical evaluation of clients, which includes blood glucose monitoring, blood pressures, and other basic health screenings Provide medication services for clients who require such services including client MARS chart reviews, injections, and blood draws as needed. Evaluate treatment effectiveness with agency medical/clinical team for voluntary and involuntary clients. Revise treatment documentation as needed Provide individual treatment sessions when appropriate to emergency needs and per prescribed treatment plan. Assist in discharge planning by providing written summaries of treatment as needed for member referrals and releases of information. Coordinate with agency prescribers to ensure refills and other communications with pharmacies. Provide immediate crisis intervention and resolution as needed. Visually supervise medication distribution as necessary Provide consultation, education, and training to the staff at Community House and assigned contract employees as needed Maintain client records in keeping with agency, county, and state guidelines Maintain member and staff confidentiality at all times. Attend scheduled meetings and conferences as assigned and in meeting requirements for RN/LPN licensing Other duties as assigned QUALIFICATIONS Active WA State RN or LPN license Current First Aid/CPR certification or ability to obtain within 90 days of hire Experience working with people with severe and persistent mental illness Must hold a valid driver’s license for 1 year with clean driving record in order to use agency vehicles.
