You will work closely with a multidisciplinary team to ensure that patients receive the highest standard of care, tailored to their individual needs.Registered Nurse Job RequirementGraduate of an accredited program for nursing education, BSN preferred.Current licensure to practice professional nursing in the State of Ohio.Current American Heart Association (AHA) Healthcare Provider CPR (BLS) certification required.Valid Ohio's Driver License and maintain motor vehicle insuranceRegistered Nurse Key ResponsibilitiesConducts an initial assessment of the patient and family to identify home care needs, including a thorough physical assessment and history of current and past illnesses.Regularly reassesses the patient's nursing needs to ensure care is adjusted as necessary.Develops and implements an individualized care plan tailored to each patient's needs, making revisions as required.Completes patient charting by the start of each workday or within 72 hours of the visit.Ensures OASIS charting is completed within three (3) days of the visit.Utilizes health assessment data to establish accurate nursing diagnoses.Creates a care plan that sets goals based on nursing diagnoses and integrates therapeutic, preventive, and rehabilitative nursing actions, involving both the patient and family in the planning process.Initiates appropriate preventive and rehabilitative nursing procedures, administering medications and treatments as prescribed by the physician.Provides counseling to the patient and family to support their nursing and related needs.
