Value-Based In-Home Primary Care NP (Geriatrics)

HarmonyCares seeks a Nurse Practitioner to deliver in-home primary care to high-acuity patients in select markets. The candidate will practice medicine with a patient-centered approach, prioritizing quality care over volume. Ideal applicants have NP licensure, DEA eligibility, CPR/BLS, and board certification in family or geriatrics, plus geriatric/home health experience.
