Work Environment

This position is onsite and involves traveling to patients' homes in the Carteret area, which allows for a flexible work environment that adapts to the needs of our clients. This role is integral to our home care team, where you will work closely with Physical Therapy Assistants and other healthcare professionals to ensure a continuous and comprehensive care approach.

Key Responsibilities

• Direct physical therapy treatment focused on developing, improving, or restoring physical function and relieving pain, aligned with each client's individualized plan of care.
• Evaluate clients through diagnostic and prognostic assessments to establish their current functional abilities and tailor therapy accordingly.
• Oversee and provide guidance to Physical Therapy Assistants, ensuring the effective execution of treatment plans and adherence to professional standards.
• Assess and recommend modifications to the home environment that promote independence and enhance rehabilitation outcomes.
• Administer therapeutic exercises, traction, massage, and other physical therapy techniques as prescribed by physicians.
• Engage with clients and their families, educating them on therapy protocols, device usage, and practical applications for daily living.
• Contribute to the interdisciplinary team by participating in case conferences, providing insights and updates on client progress and response to treatment.
• Document medical records meticulously, including initial evaluations, progress notes, and discharge summaries, ensuring compliance with agency standards.
• Maintain timely communication with the healthcare team regarding changes in client condition and modification of care strategies as necessary.
• Support the continuous improvement of our healthcare services by participating in staff training and development initiatives.

Patient Population / Client Focus

Our patient population consists primarily of individuals recovering from surgical procedures, managing chronic conditions, and those requiring rehabilitative care due to injury or age-related disabilities. Each client receives personalized attention focused on their specific needs, ensuring that every treatment session contributes to improved functional capabilities and overall well-being.

Qualifications

• Doctorate in Physical Therapy (DPT) and current license to practice physical therapy in New Jersey.
• At least two years of experience in a clinical setting, preferably in home health care or a similar environment.
• Current CPR certification and valid driver's license with appropriate automobile insurance.
• Proficient knowledge of relevant federal, state, and agency regulations governing physical therapy practices.
• Strong commitment to delivering high-quality patient care and enhancing patient experiences.

Preferred Experience

• Experience in home health care with a focus on geriatric clients is highly desirable.
• Demonstrated ability to work collaboratively within a multidisciplinary team.

Compensation and Benefits

We offer a competitive annual salary ranging from USD 100,000 to 120,000, based on experience and qualifications.
