A leading skilled nursing and rehabilitation organization is seeking an experienced MDS LPN to join its clinical team in a full-time remote position .

This is an excellent opportunity for an experienced MDS professional who is highly organized, detail-oriented, and confident managing MDS responsibilities independently.

• Location: Milford, MA 01757

• Work Setting: 100% Remote

• Employment Type: Full-Time

• Compensation: $8,200 per month

• Experience Required: 3+ years of MDS experience

• Industry: Skilled Nursing / Long-Term Care

• Complete accurate and timely MDS assessments

• Review clinical documentation and resident records

• Coordinate MDS-related information with the interdisciplinary team

• Ensure documentation supports accurate MDS completion and coding

• Monitor assessment schedules and required deadlines

• Identify documentation discrepancies and follow up with appropriate clinical staff

• Support compliance with applicable MDS and regulatory requirements

• Maintain accurate, organized, and complete clinical documentation

• Current and active LPN license

• 3+ years of MDS experience required

• Strong knowledge of the MDS assessment process

• Skilled Nursing / Long-Term Care experience preferred

• Strong clinical documentation and organizational skills

• Excellent attention to detail

• Ability to manage deadlines and multiple priorities

• Strong written and verbal communication skills

• Comfortable working independently in a remote environment

• $8,200 monthly compensation
