Oak Street Health is seeking a bilingual Community Health Worker to bridge patients, the community, and medical systems. You will conduct proactive outreach, promote health literacy, and connect patients with resources to remove barriers to healthier lives. You will collaborate with Medical Social Workers, document encounters in the EMR, and travel across Chicago-area communities to meet patients in centers, homes, or community sites.
