Responsibilities: Provide comprehensive physical therapy evaluations and treatment to patients in an outpatient setting Develop individualized treatment plans based on patient needs, diagnoses, and functional goals Perform therapeutic exercises, manual therapy, and other appropriate physical therapy interventions Monitor patient progress and adjust treatment plans as needed Educate patients and caregivers on exercises, mobility, injury prevention, and home treatment plans Maintain accurate and timely clinical documentation Collaborate with physicians and other healthcare professionals to ensure coordinated patient care Help patients improve strength, mobility, function, and overall quality of life Follow established clinical, regulatory, and documentation standards Qualifications: Doctor of Physical Therapy (DPT) or equivalent accredited Physical Therapy degree Active New York Physical Therapist license Active New York Medicare and Medicaid provider IDs — required Strong clinical and assessment skills Excellent communication and interpersonal skills Compassionate, patient-centered approach Ability to work effectively both independently and as part of a multidisciplinary team Strong organizational and documentation skills Commitment to providing high-quality care to a diverse patient population Competitive hourly compensation commensurate with experience Full-time, on-site position Collaborative and supportive clinical environment Opportunity for meaningful patient impact and long-term professional growth #J-18808-Ljbffr US Career Partners
