Job Summary The Case Management Coordinator is responsible for all discharge plan executions prepared and documented by the RN Case Manager or Social Worker. This role manages discharge arrangements and/or transfer from the facility, working closely with the RN Case Manager and Social Worker to prepare patients and families for pending discharge to appropriate care settings. The coordinator uses the post‑acute referral system for all patient placements and documents choices offered to patients. They ensure that all required documentation is forwarded to the receiving facility for discharges or transfers, manage insurance requests, and maintain MIDAS records. The position reports to the Director of Case Management. Responsibilities Execute the discharge plan for patients with post‑acute needs, completing timely and accurate arrangements for the patient’s discharge or transfer from the facility. Integrate knowledge of patients’ insurance benefits and eligibility when coordinating post‑acute needs; verify benefit coverage and work closely with payers to obtain authorization for discharge medications and post‑acute service providers (e.g., SNF, HHA, DME). Explain post‑acute choices to patients/families and communicate referral requests using the Curaspan system for all referrals. Ensure all required documentation is sent to the receiving facility for discharges or transfers. Document all work in the MIDAS Case Management system and print discharge planning notes at least daily for patient charts. Perform other related job tasks or responsibilities as assigned. Qualifications Special Skills: Communication, care coordination, teaching, proficiency in Microsoft Office. Minimum Requirements: Experience: Two years of health‑care experience. Education: High school diploma. Preferred Requirements: Experience: Two years of health‑care experience (additional experience may be considered). Education: Associate’s or bachelor’s degree.
