Key Responsibilities

• Coordinate patient care across the continuum to ensure quality outcomes.
• Perform utilization review and support efficient resource management.
• Develop and implement safe, effective discharge plans.
• Collaborate with interdisciplinary teams to optimize patient care.
• Maintain accurate electronic documentation and demonstrate strong computer literacy.

Required Qualifications

• Active New York State Registered Nurse (RN) License .
• Primary Source Verification of RN License and Registration from the New York State Department of Education (NYSED) .
• BLS Certification .
• PRI and Screen Certification (required at submission).
• Minimum 5 years of clinical experience as a Registered Nurse.
• Experience in Care Coordination, Utilization Review, and Discharge Planning .
• Associate Degree in Nursing (ASN/ADN) from an accredited nursing program.
