Bilingual Case Manager

Description Under general supervision, the Bilingual Case Manager is responsible for providing comprehensive case management services to unaccompanied children (UC) in the care of a federal program. This includes assessing needs, developing service and safety plans, facilitating family reunification, and ensuring that all services are delivered in compliance with federal, state licensing, and agency policies. Bilingual Spanish is required. The Case Manager plays a vital role in supporting each child’s safety, well‑being, and timely release to a suitable sponsor, using a trauma-informed, child-centered, and culturally responsive approach. Duties And Responsibilities Conduct initial intake and needs assessments for each child upon admission. Develop individualized service and safety plans tailored to each child’s emotional, psychological, educational, and cultural needs. Facilitate ongoing case management services that support the child’s adjustment, progress, and overall well‑being while in care. Coordinate with interdisciplinary team members (clinical, education, medical, and youth care staff) to ensure a holistic approach to service delivery. Conduct regular case reviews and update service plans as needed. Initiate and maintain regular contact with identified sponsors to assess suitability for reunification in accordance with federal/agency standards. Conduct sponsor assessments, background checks, and home studies as required by federal/agency policy. Prepare and submit release requests and documentation to federal/agency for approval. Facilitate family communication and provide guidance on reunification expectations, rights, and responsibilities. Coordinate post‑release planning and referrals to ensure continuity of care and community support. Maintain accurate, complete, and timely case records in the federal/agency‑designated database (e.g., UC Portal) and agency systems. Document all case actions, contacts, assessments, and services in accordance with federal and agency requirements. Ensure case files meet federal, agency, and state licensing standards for content, organization, and confidentiality. Submit case progress reports, release recommendations, and other required documentation by established deadlines. Participate in audits, monitoring visits, and internal compliance reviews as requested. Advocate for each child’s best interests in alignment with trauma‑informed and culturally responsive care principles. Collaborate with clinicians, teachers, youth care staff, and medical professionals to ensure coordinated care delivery. Participate in multidisciplinary team (MDT) meetings to discuss case progress and address barriers to reunification. Coordinate with external partners, including child advocates, legal service providers, and Federal Field Specialists (FFS). Accompany children to court hearings, medical appointments, or sponsor reunifications when required. Identify and report any safety concerns, incidents, or signs of abuse, neglect, or trafficking in accordance with federal/state and state mandatory reporting laws. Participate in safety planning and crisis intervention efforts as part of the program team. Ensure all case management activities promote a safe, stable, and supportive environment for the children in care. Participate in required federal/state and agency training, including those related to trauma‑informed care, cultural sensitivity, child rights, and documentation standards. Stay current with federal/state policy updates and best practices in child welfare and refugee resettlement. Support new case managers and assist in peer learning as directed by the Lead Case Manager or Supervisor. Participate in and provide support groups and classes to the community and clients of the agency. (based on position) Maintain a welcoming environment for families and other community partners. Ensure compliance with HIPAA and PII privacy and security requirements. As with all of our positions, we work as a team and various duties may be assigned at any time including tasks to support other AGH programs/services, cleaning, organizing, child care, attending to the needs of the child in your care or in the office, attendance at meetings, transportation, paperwork, etc. Perform other duties as assigned. Reporting To: Lead Case Manager Hiring Rate of Pay: $31.25 per hour Requirements To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Bachelor’s degree in Social Work, Psychology, Sociology, or other relevant behavioral science, human services or social services fields. Child welfare and/or case management experience is strongly encouraged. 2–3 years of experience in case management, child welfare, family reunification, or social services. Strong understanding of child welfare principles, immigration systems, and family reunification processes. Bilingual Spanish required. Excellent communication, assessment, and documentation skills. Ability to work with diverse populations and maintain professionalism in high‑stress situations. Strong computer literacy, including Microsoft Office and data management systems. Be able to obtain Criminal and Child Abuse Clearance from the State of California/Department of Justice. Must be available full‑time and able to adjust work day/work week to provide services when clients and families need them, which can routinely include evenings and occasionally weekends, in order to ensure coordination and continuity of services. Demonstrate the Understanding of the importance of compliance with HIPAA privacy practices. Ability to work independently and as part of a team, strong conflict resolution, communication and organizational skills; with great attention to detail. Ability to adhere to our attendance and punctuality policy. Must pass and remain current in CPR/First Aid. Must be able to effectively utilize computers and basic computer software. Must meet Licensing (or contract if not FFA position) and insurance requirements. Must have a valid Driver’s License and a good driving record. Must be at least 18 years old with at least 2 years of driving experience. A Greater Hope includes all employees in the DMV pull notification process, notifying AGH of infractions throughout the process. All Employees are required to report all arrests within 48 hours of arrest as per Licensing requirements. Must pass a drug screen, physical exam, and Tuberculosis skin test. A Greater Hope participates in E‑Verify and will verify your Social Security Number. Foster Family Agency experience preferred. (if applicable) Additional Qualifications This position is funded through a federal cooperative agreement. Continued employment is contingent upon continued grant funding, federal/agency approval (when applicable), and continued compliance with all federal/agency, licensing, and A Greater Hope personnel requirements. As a Condition Of Employment, The Successful Candidate Must Meet all minimum qualifications established by federal/agency and A Greater Hope for the position. Successfully obtain and maintain a Federal/Agency Suitability Determination, including all required background investigations and clearances, prior to having contact with unaccompanied children. Successfully complete all required criminal history background checks, child abuse and neglect registry checks, fingerprint‑based clearances, and other background screenings required by federal/agency, Community Care Licensing (CCL), and A Greater Hope. Successfully complete all required Federal/Agency pre‑service, onboarding, annual, and ongoing training within required timeframes. Demonstrate the ability to provide trauma‑informed, culturally responsive, and child‑centered services to unaccompanied children. Bilingual proficiency in English and Spanish is required for positions designated as bilingual and may be verified through A Greater Hope's language proficiency assessment process. Provide documentation of all required immunizations and authorize A Greater Hope to obtain and verify immunization records through the California Immunization Registry (CAIR), as applicable. Successfully complete and maintain all required health screenings, physical examinations, tuberculosis (TB) clearance, immunizations, vaccinations, and other medical requirements established by federal/agency, Community Care Licensing (CCL), applicable law, or A Greater Hope policy. Maintain all required licenses, certifications, registrations, and credentials applicable to the position throughout employment. Comply with all applicable federal/agency policies, federal regulations, state laws, licensing requirements, accreditation standards, confidentiality requirements, and A Greater Hope policies and procedures as a condition of employment. Possess and maintain authorization to work in the United States. Positions requiring transportation duties must maintain a valid driver's license, an acceptable driving record, and insurability under A Greater Hope's automobile insurance policy. The employee may be working outside for periods of time in all seasons of weather. Employees will be required to drive a motor vehicle. This position requires regular and consistent attendance, excessive unplanned absences are disruptive to our Agency business. Therefore, each employee must at all times be emotionally stable and able to function effectively with children, adolescents and adults who may have mental or behavioral health problems.
