Nurse Practitioner or Physician Assistant (PRN) - In-Home Health Assessments

Overview $2,000 Sign‑On Bonus. Matrix Medical Network is a national in‑home clinical network providing comprehensive health assessments. We serve older adults and other at‑risk individuals, delivering personalized care that improves health outcomes and identifies chronic conditions. Responsibilities Conduct 1‑hour adult/geriatric assessments covering medical history, diagnosis, treatment, health education, physician referrals, case‑management referrals, follow‑up, and documentation per Matrix guidelines and protocols. Administer and order point‑of‑care and lab‑based testing as needed. Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel. Work with the Primary Care Physician on patient education and follow‑up. Provide services at the member’s residence, including homes or Skilled Nursing Facilities. Benefits of Working at Matrix Flexibility: Visits are scheduled based on your availability, from 8 a.m. to 7 p.m., Sunday through Saturday. You can work during the day, evening, and/or weekend. Competitive Pay: Earn income based on the number of assessments completed. Monthly earnings range from $1,800 to $11,500 (approximately $90–$115 per assessment) depending on your state of residence. We also provide comprehensive onboarding training, a preceptor, 1:1 feedback, and free continuing education. We support additional state licensure and credentialing. Full‑time Opportunities: PRN providers may transition to full‑time roles at their home location or region and receive a full‑time sign‑on bonus. Qualifications Master’s degree or equivalent experience and satisfactory completion of NP licensure. Current NP licensure in state of practice, including prescription authority or ability to obtain it. AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty. Current BLS, ACLS, or CPR certification. 1 year of NP experience preferred; new graduates encouraged. Valid state driver's license and automobile insurance coverage for the state of residence. Own the required liability insurance: Bodily injury $100,000 per person/$300,000 per incident; Property damage $50,000; Collision and comprehensive deductibles $1,000 or less. Strong computer skills and familiarity with employee health/medical record software. Excellent verbal and written communication skills. Licensure and Insurance State licensure: Valid NP license in the state of practice. Insurance: Liability coverage as specified above.
