Case Manager

Susquehanna County Interfaith is a nonprofit organization dedicated to supporting individuals and families through various community-oriented programs. These programs include Christmas and Thanksgiving assistance, fuel and emergency aid, a school shoe program, and clothing distribution. The organization also operates a thrift store, providing affordable clothing and household items to the community. Interfaith is committed to offering resources and support to those in need within the Susquehanna County area. Role Description This is a full‑time, on‑site role as a Case Manager at Susquehanna County Interfaith, located in Susquehanna, PA. The Case Manager will be responsible for assisting clients in accessing resources and services, conducting needs assessments, maintaining accurate records, and developing action plans. The role also involves building strong relationships with clients, supporting community outreach programs, and collaborating with other team members to ensure effective service delivery. Qualifications Strong organizational and case management skills, including the ability to prioritize and manage multiple responsibilities effectively Excellent verbal and written communication skills, including the ability to communicate effectively with clients and community partners Proficiency in administrative tasks such as maintaining accurate records, generating reports, and handling confidential information Interpersonal skills, empathy, and the ability to build trusting relationships with individuals from diverse backgrounds Experience or familiarity with working in a nonprofit or social services setting is highly advantageous Bachelor’s degree in Social Work, Human Services, or a related field preferred, or equivalent relevant experience Proficiency in common computer applications, including Microsoft Office Suite Understanding of local resources and programs is a plus #J-18808-Ljbffr
