Social Worker Assistant

Job Summary Under the supervision of the Social Work Supervisor, the Social Worker Assistant will provide social work services to participants of Hong Fook Centers. Responsibilities include intake of prospective participants, language assistance, collaboration with team members for care management, and assistance with psycho-social assessment. Major Duties and Responsibilities Introduce Hong Fook Adult Day Health Care programs to prospective participants and their families. Perform intake, home visits, and initial assessments of potential participants. Follow up to bring participants in for program assessment, including obtaining medical reports, scheduling transportation, and notifying team members. Enroll participants by explaining program rules and rights, and assist new participants’ adjustment. Provide care management services to participants. Act as liaison to assist participants in obtaining needed services and benefits. Interpret service or benefit notices and medical insurance statements for participants or families. Assist the Social Work Supervisor with periodic assessments of program participants and maintain written records in compliance with licensing requirements. Help conduct support groups for program participants. Participate in outreach activities targeting frail elderly and disabled adults. Serve as a member of the multidisciplinary team. Perform other appropriate tasks as directed by the supervisor. Qualifications BA in Social Work or related field. Experience and interest in working with frail elderly or disabled populations. Ability to work in a diversified environment. Ability to work independently and as part of a multidisciplinary team. Good oral and written communication skills. Public presentation experience is a plus. Bilingual in English and Cantonese/Mandarin desirable; knowledge of one of the targeted Asian languages (Vietnamese, Filipino, other Southeast Asian languages, or Korean) desirable. Flexibility in work shifts to meet program needs. Valid California driver license and reliable automobile desired. Ability to handle multiple tasks. ADA Requirements Ability to communicate clearly with others. Ability to see and hear to interact with others. Capability to lift a minimum of 20 lbs. Salary Range The target hourly rate for this position is $25.30 – $30.50 with benefits. Compensation will be determined based on education, experience, and skill set of the selected candidate.
