About the Role:

As an On-Call Hospice RN at Divine Guardian LLC, you will play a vital role in providing compassionate care to patients in their final stages of life. Join our dedicated team in Tucker, GA, and make a meaningful impact on families during their most challenging times.

Responsibilities:

• Provide high-quality, patient-centered nursing care to hospice patients in various settings.
• Conduct comprehensive assessments and develop personalized care plans.
• Administer medications and manage symptom control effectively.
• Collaborate with interdisciplinary teams to ensure holistic care.
• Educate patients and families about end-of-life care options and resources.
• Document patient care and maintain accurate medical records.
• Respond promptly to patient and family needs, ensuring comfort and dignity.
• Participate in on-call rotations to provide support as needed.

Requirements:

• Current RN license in the state of Georgia.
• Minimum of 1 year of nursing experience, preferably in hospice or palliative care.
• Strong assessment, communication, and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate demeanor with a focus on patient-centered care.
• Basic Life Support (BLS) certification required.
• Knowledge of hospice regulations and standards of care.
• Flexibility to work on-call hours as needed.
