Physical Therapist

The role offers the autonomy to manage treatment plans, build strong relationships with patients and families, and help individuals improve their mobility, independence, and overall quality of life. What your day will look like: Oversee and coordinate the delivery of physical therapy services to patients in the home health setting. Perform comprehensive physical therapy evaluations and provide treatment interventions based on physician orders and individualized patient needs. Delegate appropriate therapeutic activities to Physical Therapist Assistants and provide ongoing direction and supervision. Educate and support therapy team members regarding treatment plans, patient precautions, expected outcomes, and follow-up care requirements. Develop individualized care plans with measurable short-term and long-term objectives, monitor patient progress, and adjust treatments as needed in collaboration with the physician.
